Sample Written Safety Program Templates
BWC’s Division of Safety and Hygiene (DSH) has created sample written safety program templates for employers to download and to customize to their operations. DSH utilized our safety experts from Loss Prevention Field Operations, including specialized consultants from the OSHA On-Site Consultation Program, and the Public Employment Risk Reduction Program to create an assortment of branded sample written program templates to share with employers.
Each template is laid out identically and has a welcome page with information on how to use the template. DSH Assistant comments provide added guidance, best practices, or links to resources or reference standards. Additionally, each sample written program template offers supporting documents for training, checklists, definitions, and more. Specific program elements and training information are located in separate sections of each template.

Call Before You Dig
By law, everyone must contact the Ohio Utilities Protection Service at least 48 hours but no more than 10 working days (excluding weekends and legal holidays) before beginning any excavating* or digging project.
A vital resource for Ohio residents and businesses alike, the Ohio Utilities Protection Service (O.U.P.S.) acts as a communication link between utility companies and individuals planning any digging activity. The Ohio Utilities Protection Service can be reached at 8-1-1 or 1-800-362-2764.
*Please note that the Ohio Revised Code defines excavator as “the person or persons responsible for making the actual excavation” (ORC Sec. 3781.25 (K)). Therefore, while O.U.P.S. will not refuse a ticket from any caller, we advise the actual excavator place the call to O.U.P.S., receive their own ticket number and wait the required 48 hours in order to be compliant with Ohio law.
